Job summary

Transition Home Healthcare & Pathways Hospice is seeking a compassionate Hospice CNA to join our patient-centered team in Houston. This PRN position supports the nursing staff, helps with daily living activities, and maintains accurate patient records under the direction of the case manager.

Requirements include Texas CNA license, at least 1 year of hospice experience, ability to lift up to 50 pounds, and flexibility for weekends/holidays/evenings. Experience with HCHB EMR is a plus.

Qualifications

• Texas CNA license is required.
• One year of hospice experience is required.
• Able to lift up to fifty pounds.
• Able to work weekends, holidays, and evenings.
• Strong communication and people skills.

Responsibilities

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
• Help with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review any important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
