Job summary

Transition Home Healthcare is seeking a compassionate Hospice CNA in Central Houston (Inner Loop) for PRN shifts. You will join a patient-centered team delivering high-quality care in coordination with the nursing staff.

Responsibilities include assisting with ADLs, maintaining records, reporting changes to the case manager, educating patients and families, and documenting notes. Texas CNA license and hospice experience preferred.

Qualifications

  • Must live in Central Houston (Inner Loop)
  • CNA license in Texas (required)
  • One year of hospice experience
  • Able to lift up to 50 pounds
  • Able to work occasional weekends, holidays, evenings
  • Strong communication and people skills
  • Experience with HCHB EMR (preferred but not required)

  • Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
  • Help with patient meals, feeding, and basic activities of daily living.
  • Deliver weekly reports to the case manager and review any important changes in patient conditions.
  • Educate patients and family members.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate effectively with healthcare providers and other care team members.

Hospice CNA

Shift Available: PRN

Service Area for Coverage: North, Northwest, South, Southeast
