Job summary

HopeWest is seeking a Certified Nurse Assistant (CNA) for Hospice in Delta and Montrose, CO. This PRN-Variable position provides personal care and homemaker services to patients under RN supervision, engaging with a multidisciplinary team to deliver compassionate care.

Requirements include a Colorado CNA license and a Colorado driver’s license with reliable transportation if providing community care. Compensation ranges and benefits are described in the posting.

Qualifications

• Current Colorado CNA License.
• Valid Colorado Driver’s license, adequate automobile insurance and reliable transportation, if providing care in the community setting.
• A genuine spirit of caring and compassion.
• Effective communication and ability to work within a multidisciplinary team.

Responsibilities

• Provide personal care and homemaker services under the supervision of the Registered Nurse.
• Be part of an interdisciplinary team delivering patient- and family-centered care.
