Job summary

Three Oaks Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team serving patients in their homes across Illinois. Primary function is to provide personal healthcare and related services, ensure a safe, clean environment, and collaborate with patients, families, and the supervising RN.

Requirements include CNA license, High School diploma preferred, and current driver's license with at least one year of hospice or similar experience preferred.

Qualifications

• Must have CNA license.
• Has completed nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure programthat meets requirements.
• High School Diploma preferred.
• Must have current driver's license.
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.

Responsibilities

• Provide personal healthcare and related services to the patient in their place of residence.
• Assist in providing a safe and clean environment, work cooperatively with patient and family.
• Share observations and problems with the supervisor; works under the direction and supervision of a registered nurse.
