Qualifications

• Current GA CNA certification.
• Minimum 1 year hospice/palliative care CNA experience.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout the service area.
• Strong communication and teamwork skills.
• Compassionate, patient-focused approach to care.

Responsibilities

• Assist patients with activities of daily living.
• Provide mobility assistance and transfers.
• Offer companionship and emotional support.
• Follow patient care plans and infection prevention standards.
• Document care timely and accurately.
• Coordinate with RN case manager and hospice team.
• Maintain a clean, safe home care environment.
