Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant to provide direct patient care on the road. You will perform daily living activities, review care plans, assist with transfers, mobility, nutrition, and documentation under an RN Case Manager.

Required CNA license and 12 months of caregiving experience; CPR preferred. Strong communication, time management, and professional boundaries are essential for delivering dignified end-of-life care.

Qualifications

• CNA license required.
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver’s license required.
• Valid Auto Insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
