Inspire Hospice & Palliative Care is seeking a compassionate Hospice Care Assistant (CNA) to join our dedicated caregiving team. You will support patients with daily living activities, provide comfort, and coordinate with the RN case manager to deliver high-quality end-of-life care.

Qualifications

• Current CNA certification in Ohio.
• Minimum of one year of experience in CNA, hospice, palliative care, or similar setting.
• Valid driver''s license and reliable transportation.
• Clean driving record and current auto insurance.
• Ability to travel throughout the assigned service area.
• Strong communication, teamwork, and interpersonal skills.
• Compassionate, patient-focused approach to care.

Responsibilities

• Provide compassionate patient care: assist with daily living activities, transfers, mobility, companionship, and dignity.
• Observe, document changes and communicate concerns to supervising nurse.
• Create a peaceful, safe environment and provide compassionate presence to patients and families.
• Coordinate with RN case manager and hospice team for individualized care.
• Communicate condition changes to clinical team.
• Build trust with patients, families, caregivers, and hospice team.
• Follow patient plan of care and perform duties within scope.
