Job summary

Hospice of the Piedmont, Inc. is seeking a full-time, hourly, non-exempt CNA to provide personal care and home management for patients in their homes within a 30-mile radius of our High Point office.

The role requires a CNA with at least 1 year of experience and a valid Driver's License; CHPNA is preferred. Schedule is Monday–Friday, 8am–5pm, with 4–6 visits daily and supervision by an RN Case Manager.

Qualifications

• CNA certification required.
• 1 year CNA experience preferred.
• Driver's license required.

Responsibilities

• Follows nurse aide care plan to provide personal care and home management.
• Reports changes in patient condition to nursing staff promptly.
• Collaborates with IDG and other departments to meet patient goals.
• Documents patient care clearly and timely per policy.
• Manages time, supplies, and equipment effectively.
• Arranges and adheres to visit timeframes with patients and facilities.
• Maintains a clean, safe, and comfortable home environment.
• Teaches home safety, energy conservation, and sanitation; provides light meal prep.
• Demonstrates strong communication and builds rapport with patients and families.
• Complies with nurse aide standards; participates in staff training.
• Average of 4-6 visits per day.
