Job summary

Three Oaks Hospice, Inc. is seeking a Certified Nursing Assistant (CNA) to provide compassionate in-home patient care. You will assist with daily living activities, monitor health status, and support families while working under the supervision of a registered nurse.

The ideal candidate will have a CNA license, a high school diploma (preferred), and reliable transportation. Hospice experience is a plus as we deliver high-quality end-of-life care in patients' homes.

Qualifications

• High School Diploma preferred.
• Must have CNA license / state certification.
• Must have current driver’s license.
• At least one year experience in hospice or related field preferred.

Responsibilities

• Provide personal healthcare to patients in their home.
• Maintain a safe and clean living environment.
• coordinate with supervising RN and family members.
• Document observations and report concerns to supervisor.
