Your Health Organization is seeking a dedicated Certified Nursing Assistant (CNA) to service hospice patients across designated SC service areas. The role involves in-home and ALF visits, delivering medications and DME, and coordinating telehealth with pharmacists and providers.

Valid CNA certification and reliable transportation are essential, with full-time hours on weekdays. Responsibilities include assisting with ADLs, monitoring patient conditions, and maintaining accurate documentation.

Qualifications

• Certified Nursing Assistant certification in good standing.
• Phlebotomy experience preferred; willing to train if not experienced.
• Experience with home health or public health is preferred.
• Experience delivering DME or managing logistics is a plus.
• Proficiency with technology tools for remote patient monitoring and telemedicine.

Responsabilités

• Provide personal care to hospice patients in homes and ALFs, respecting dignity and comfort.
• Assist with mobility, transfers, and fall-prevention support.
• Perform light housekeeping related to patient care and maintain cleanliness of care areas.
• Monitor and document patient condition changes and report to clinical team promptly.
• Deliver medications and review instructions; reconcile medications.
• Coordinate telehealth visits between patients and pharmacists, therapists, and providers.
• Collect blood specimens during visits and perform routine venipuncture.
• Ensure timely access to care and necessary supplies (DME) for patients.
