Job summary

Optimal Care - Evergreen is seeking a compassionate Hospice Aide/Homemaker to support patients at home, helping with personal care, meals, and mobility while maintaining dignity and comfort. The role involves daily travel within the Allegan, Ostego, Wayland, Fennville, Douglas, Kalamazoo area and collaboration with nurses and the care team.

Qualified candidates will have CNA certification, 75 hours classroom training plus 16 hours practical, reliable transportation, and a valid driver’s license.

Qualifications

Must read and write English; follow written/verbal instructions.
Must hold current CNA certification and be in good standing.
Reliable transportation and valid license/insurance.

Responsibilities

Provide personal care (bathing, dressing, grooming, toileting) to patients at home.
Assist with meals and mobility to maintain quality of life.
Travel to patients' homes, assisted living facilities, or nursing homes daily.
Collaborate with nurses and interdisciplinary care team to support patient needs.
