Job summary

Optimal Care is seeking a compassionate Hospice Aide/Homemaker to support patients in their homes. You will assist with personal care, mobility, meal prep, and comfort, coordinating with nurses and the care team to maintain quality of life.

This role involves daily travel within Allegan, Ostego, Wayland, Fennville, Douglas, and Kalamazoo areas, and requires empathy, reliability, and strong communication. The position offers mileage reimbursement, PTO, medical/dental/vision insurance, HSA/FSA

Qualifications

• Completed a minimum of 75 hours of classroom and supervised practical training.
• Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training.
• Must hold current certification as a nurse aide and be in good standing on the state nurse aide registry.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities

• Assist with personal care including bathing, dressing, grooming, toileting, oral hygiene.
• Assist with mobility and activities of daily living to support patient comfort and independence.
• Prepare meals and assist with feeding as needed while maintaining patient dignity and safety.
• Collaborate with nurses and the interdisciplinary care team to tailor care to each patient.
