Job summary

Advantage Home Health and Hospice in Altoona, PA, seeks a dedicated Certified Nursing Assistant (CNA) to join our hospice care team. This full-time daytime role provides compassionate in-home patient care, support for families, and collaboration with the interdisciplinary team.

Daylight shift 6am-2:30pm and a strong emphasis on patient comfort and dignity. The ideal candidate will hold a CNA certification, have a valid driver's license with auto insurance, and demonstrate empathy, communication,

Qualifications

Valid Driver's license and state-required auto insurance.
Empathy and compassion for patients and families.
Excellent communication and listening skills.
Ability to provide physical and emotional comfort to those in need.
Collaboration with the hospice care team.
Understanding of end-of-life care principles; hospice experience a plus.

Responsibilities

Deliver empathetic and compassionate care to patients in their homes or facilities.
Assist patients with activities of daily living, pain management, and symptom control.
Provide emotional support and companionship to patients and their families.
Coordinate care with the interdisciplinary hospice team.
Maintain accurate patient records and documentation.
Ensure a peaceful and comfortable environment for patients.
Act as an advocate for patients and their families.
