Job summary

Chapters Health System is seeking a CNA/Home Health Aide to provide personal care and homemaker assistance under RN supervision in patients\' homes throughout the Largo, FL area. The role requires current CNA certification, a valid driver’s license, BLS, and the ability to communicate in English.

Qualifications

• High school diploma or GED or equivalent
• Current CNA certification in the state of employment
• Minimum six months CNA/Home Health Aide experience preferred
• Active BLS for healthcare professionals (American Heart Association or Red Cross)
• For current Hope Employees: BLS by Jan 1, 2025
• For current Capital Caring employees: BLS by Nov 1, 2024
• Valid driver’s license and automobile insurance per Company policy
• Reliable transportation to meet work schedule
• Ability to use equipment with visual and auditory mechanisms
• Ability to communicate in English
• Ability to manage emotional stress of caring for ill patients and families
• Must be able to perform essential functions including lifting and pushing/pulling with/without accommodations
• Must be able to access homes/facilities in assigned area (stairs, tight spaces, heat)
• Must be willing to have contact with individuals with communicable diseases

Responsibilities

• Represent Company professionally through care delivered and services provided to clients
• Comply with all regulations and maintain strong fraud/abuse protection

• Provide patient/caregiver services per Plan of Care and policies
• Practice safe body mechanics when moving patients
• Keep homes clean and safe for patients
• Provide caregiver relief as part of Plan of Care
• Transport supplies and samples as needed under RN direction
• Contribute to Plan of Care by reporting to RN
• Communicate issues to RN/Clinical Manager
• Be available for emergency deployments as required
