Qualifications

• Completion of a nursing assistant or patient care tech program beyond high school.
• Basic computer skills and ability to document care.
• Familiarity with office equipment and data entry.

Responsibilities

• Provide personal care and comfort to hospice patients.
• Assist with bathing, grooming, nutrition and mobility.
• Record vital signs and report changes to RN.
• Perform homemaker tasks to promote home safety and cleanliness.
• Assist with end-of-life care and bereavement support as needed.
• Support agency operations and maintain cleanliness of storeroom.
