Job summary

Interim HealthCare is seeking a PRN Hospice CNA to support patients in Charleston, Berkeley and Dorchester counties. You’ll assist with bathing, dressing, meals and companionship, delivering compassionate home-based hospice care.

Follow care plans, help with ADLs and mobility, provide medication reminders, and maintain a safe home. Must have SC CNA certification, CPR, high school diploma, 1 year CNA experience, and a valid driver’s license with transportation.

Qualifications

• High school diploma or equivalent.
• Active SC CNA Certification.
• 1 year of CNA experience.
• Hospice experience preferred.
• CPR certification.
• Valid driver’s license and transportation.
• Ability to lift up to 50 lbs.

Responsibilities

• Provides services as stated in each patient's hospice care plan.
• Assist with activities of daily living such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation and companionship.
• Help with ambulation, transferring and range of motion exercises.
• Provide medication reminders, document their condition and notify a supervisor of any concerns.
• Ensure a safe home environment with unobstructed pathways.
• Participate in activities that bring clients joy such as puzzles, games, reading and hobbies.
