Job summary

Mercy Medical Center Home Health and Hospice is seeking a Hospice Aide I to deliver compassionate, hands-on support to patients during their end-of-life journey. Under RN supervision, you will assist with personal care, provide comfort to patients and families, and participate in care coordination.

The role emphasizes dignity, empathy, and collaborative teamwork in a home-health setting, with a PRN schedule and adherence to HIPAA and agency policies.

Qualifications

• Current CNA/STNA certification in the state of practice.
• Minimum 1 year of hospice, home health, or long-term care experience preferred.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Provide personal care to patients under the plan of care and RN supervision.
• Assist with ADLs: bathing, dressing, grooming, toileting, mobility.
• Offer emotional and physical comfort to patients and families during end-of-life care.
• Observe and document patient condition; report changes to the RN.
• Maintain a clean, safe environment and practice infection control.
• Maintain confidentiality per HIPAA and agency guidelines.
• Participate in team meetings and in-service education.
• Maintain accurate records per agency standards.
