Job summary

Episcopal Homes seeks a compassionate Hospice Home Health Aide (HHA) to provide personal care and comfort to patients receiving end-of-life care in hospice settings in Oakdale, MN. The role offers full-time day shifts with on-site responsibilities and requires close collaboration with our hospice nursing team.

Assistance with mobility, meals, and daily activities is essential, along with documenting care and adhering to individualized care plans.

Qualifications

• Meets Minnesota Nursing Assistant training requirements.
• Completion of at least 75 hours of classroom and supervised training in a nursing assistant program.
• Must be at least 18 years old.
• Understanding of hospice philosophy and comfort providing end-of-life care.

Responsibilities

• Provide personal care: bathing, dressing, grooming, hygiene.
• Assist with mobility, transfers, positioning, and ROM exercises.
• Help with meals and nutritional needs.
• Monitor patients and report changes to RN Case Manager.
• Provide skin care and prevent skin breakdown.
• Offer emotional support to patients and families.
• Assist with light housekeeping for safety and cleanliness.
• Document patient care accurately and timely.
• Collaborate with hospice interdisciplinary team to ensure quality care.
