Job summary

Episcopal Homes in Minnesota is seeking a compassionate Hospice Home Health Aide (HHA) to provide personal care and comfort to patients receiving end-of-life care. You will work with our hospice nursing team in home and community-based settings.

The role requires Minnesota Nursing Assistant training, 75 hours of instruction, and at least 18 years old; 1 year hospice experience preferred. Compensation is hourly, with daytime shifts and opportunities to support families.

Qualifications

• Meets Minnesota Nursing Assistant training requirements.
• Completion of at least 75 hours of classroom and supervised training, including clinical and practical training.
• Must be at least 18 years old.
• One year of hospice or end-of-life care experience preferred.

Responsibilities

• Provide personal care including bathing, dressing, grooming, and hygiene.
• Assist with mobility, transfers, and range-of-motion exercises.
• Help with meals and nutritional needs.
• Monitor patient condition and report changes to RN Case Manager.
• Provide skin care and prevent breakdown.
• Offer emotional support to patients and families.
• Document care accurately and timely.
• Work with hospice team to ensure quality care.
