Qualifications

• Driver license and auto insurance proof will be requested; driving is required by state law.
• At least 1 year of home health/hospice experience in relevant settings.

Responsibilities

• Provides personal care, assists with activities of daily living, and basic housekeeping.
• Performs prescribed therapeutic exercises as directed and carries out plan of care under RN supervision.
