Job summary

Hospice of the West is seeking a Per Diem Hospice Aide (CNA) to join our team, delivering hands-on end-of-life care in patients’ homes within Yavapai County. You will work closely with the nursing team to meet physical, emotional, and personal needs with compassion and professionalism.

The role offers a Per Diem schedule with comprehensive benefits and growth opportunities, plus a hands-on onboarding program to ensure a smooth transition into our team.

Qualifications

• High School Diploma or G.E.D. required.
• Current unencumbered CNA certificate in the state.
• Minimum one (1) year experience as a CNA in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Provide Personalized Care: Follow the plan of care to ensure comfort and well‑being of patients and support for families.
• Assist with Personal Care: Bathing, oral hygiene, bedding, clothing, and hygiene tasks to maintain dignity.
• Be Dedicated to Customer Service: Emphasize exceptional service and patient satisfaction.
• Provide Support for Families: Guide and support families caring for terminally ill loved ones.
• Maintain Accurate Documentation: Accurate recordkeeping per agency and regulatory standards.
