Job summary

Guardian Angel Hospice in Indiana seeks a compassionate Hospice Aide to support patients in homes and facilities under RN supervision. You will assist with ADLs, mobility, and comfort following each patient’s Plan of Care.

Ideal candidates hold a CNA or HHA certification in Indiana, have reliable transportation, and can travel within the service area while participating in on-call rotations to deliver empathetic, high-quality care.

Qualifications

• Demonstrates compassion and patient-centered approach.
• Manages time effectively to balance multiple patients.
• Able to work independently within a supportive team.
• Willingness to travel within service area and participate in on-call rotations.

Responsibilities

• Provide direct patient care and ADL support under RN supervision.
• Assist with bathing, grooming, oral hygiene, skin and nail care, and other personal needs.
• Assist with transfers and mobility using hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches.
• Report changes in patient condition to RN Case Manager and care team.
