Job summary
Heart to Heart Hospice in Beaumont, TX is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under an RN's supervision. You will support safe transfers, assist with daily activities, and document care per policy.
The ideal candidate holds an active CNA license, a high school diploma or GED, and at least one year of hospice or nursing assistant experience. A valid driver’s license and reliable transportation are required, along with a commitment to confidentiality and
Qualifications
- High School Diploma or GED preferred.
- Unencumbered CNA certification/license.
- Current CNA license.
- At least 1 year hospice/Nursing Assistant experience.
- Demonstrates compassion for ill and elderly; maintains confidentiality.
- Must have valid driver's license and reliable transportation.
- EMR experience is a plus.
- Adherence to policies and procedures.
Responsibilities
- Provide direct patient care under RN supervision according to Plan of Care.
- Assist with safe transfers and ambulation per policy.
- Report/communicate changes and document care for continuity.
- Practice infection control principles.
- Promote respectful communication with patient/family and staff.
- Timely submission of all documentation.
- Perform other duties as required.
- Ensure compliance with laws, Medicare regulations, and policies.
