Palmetto Infusion in Greenville, SC is seeking a Nurse Case Manager for Home Infusion to coordinate patient care across home infusion pharmacy, home health, hospitals, and physicians. You will provide nursing oversight from admission through therapy completion and guide patients and caregivers in managing home infusion therapies.

The role requires RN licensure, 1 year clinical experience, and ability to work M-F 8:00–5:00 with on-call responsibilities.

Qualifications

• Active Registered Nurse license in the state of practice.
• Graduate from an accredited school of nursing.
• 1 year current clinical nursing experience.
• BLS or obtain upon hire.
• Proficient with MS Outlook, Word, and Excel.
• Ability to cope with ambiguous environment while under pressure.
• Must be able to work as a team member and develop productive working relationships.

Responsabilités

• Provides infusion and other skilled nursing care to patients.
• Patient assessment and reassessment.
• Patient and caregiver education/teaching.
• Development and revision of the Plan of Care and the Plan of Treatment.
• Implementation of physician orders.
• Monitoring patient response.
• Response to emergency patient needs.
• Psychosocial support.
• Provides support and coordinated marketing efforts with sales team.
• Completes all required patient care documentation and reports accurately.
• Communicates changes to supervisor, physicians and other health care providers.
• Maintains knowledge through continuing education and training.
• Participates in staff orientation.
• Takes on-call responsibility as assigned.
• Participates in Performance Improvement processes.
• Complies with applicable laws and PI policies.
