Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN/LVN) for per diem home healthcare across patient homes in Missouri. You will provide direct patient care, implement care plans, and support families in the home setting.

This role emphasizes teamwork, compassionate communication, and reliable transportation. Active LPN/LVN license required; per diem rate is $50-$60 per visit.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
