Job summary

National Church Residences is seeking a Speech-Language Pathologist to join the Home Health & Hospice team in the Southern Ohio region. The role focuses on evaluating and treating adults with speech, language, cognitive, and swallowing disorders in patients' homes, collaborating with families and healthcare providers to develop personalized care plans.

Qualifications

Master’s Degree in Speech-Language Pathology from an accredited program.
Valid state license as required by the state Board of Speech-Language Pathology and Audiology (Ohio).
2–4 years of clinical experience preferred but not required.

Responsibilities

Evaluate and treat adults with speech, language, cognitive, and swallowing disorders in the home setting.
Develop personalized care plans in collaboration with patients, families, and healthcare providers.
Educate and counsel patients and caregivers on communication techniques and strategies.
Maintain timely and accurate documentation of services provided.
Support program development and participate in interdisciplinary care and quality improvement initiatives.
Stay current with best practices and continuing education in the field.
