Job summary

CenterWell Home Health is seeking a Medical Social Worker to evaluate and develop personalized care plans addressing patients' emotional and social needs in the home health setting. Reporting to the Clinical Manager, you will supervise the Social Worker Assistant and collaborate with the interdisciplinary care team.

You will ensure documentation, discharge planning, and adherence to company policies and healthcare regulations while providing compassionate, patient-centered support.

Qualifications

• Licensure as a social worker in the applicable state.
• Experience in discharge planning and community resources.
• Ability to read, write and speak fluent English.
• Must have CPR certification and valid driver's license.

Responsibilities

• Assess patient's social and emotional state related to illness and care needs.
• Assess relationships, financial resources, and community supports.
• Coordinate actions to obtain community resources to aid recovery.
• Communicate with patient, family and care team.
• Provide ongoing assessment of patient and family needs during treatment.
• Participate in interdisciplinary care and support other health team members.
• Evaluate Social Work Assistant performance for compliance and quality.
• Maintain and submit case documentation and progress notes.
