Qualifications

Responsibilities

• Assess patients' condition during every visit and chart observations.
• Perform evaluation tasks including vital signs and medication review.
• Dress or re-dress open wounds and assess healing progress.
• Educate patients and families on home health care strategies.
• Coordinate with therapy and healthcare professionals involved in care.
• Make recommendations for devices to improve quality of life.
• Listen to the patient and respond to concerns.
