Job summary

Argus Home Health Care is seeking an RN to provide skilled nursing care in patients' homes across the Colorado service area. You will coordinate care plans, admit patients, and collaborate with interdisciplinary teams to promote optimal health outcomes.

The role emphasizes compliance with state regulations and our core values, with responsibilities including on-call duties, discharge planning, patient education, and regular clinical documentation.

Qualifications

• Current Colorado RN License required, or compact covering Colorado.

Responsibilities

• Participate in on-call duties per policy.
• Support the Quality Management program.
• Arrange necessary equipment, supplies, and services for patients.
• Continuously review patient care per CMS and Colorado guidelines.
• Complete initial OASIS patient assessments.
• Use assessment data to establish nursing diagnoses and develop care plans with patient/family input.
• Initiate and update plans of care as patient needs change.
• Provide skilled nursing procedures, medications, and treatments as prescribed by physician.
• Supervise LPNs and home health aides per regulations.
• Reassess patients with change in condition or hospital discharge.
• Identify and implement discharge planning needs.
• Maintain knowledge of home health regulations.
• Prepare clinical notes and update physicians regularly.
• Communicate patient needs and condition changes to physicians; obtain orders as required.
• Collaborate with team members and area team leader.
• Educate patients and caregivers on self-care, medications, diet, and preventive health measures.
