Parx Home Health Care is seeking a Registered Nurse to join our team and deliver high-quality care in patients' homes and communities.

The RN will conduct assessments, develop care plans, educate patients, and coordinate with physicians and other professionals to ensure continuity of care. A Florida license is required along with CPR and a TB screen or chest X-ray, and a valid driver’s license.

Qualifikationen

• Current and unrestricted RN license in Florida.
• Strong clinical skills and ability to perform evidenced-based nursing practices.
• 1–3 years of clinical experience in home health, hospice, or community health.
• Excellent communication and interpersonal skills with patients, families and providers.
• Physical Exam and TB Test or Chest X‑Ray (no older than 6 months).
• Yearly mandatory in-service.
• Proficient in using electronic health records and healthcare technologies.
• Valid FL driver's license and reliable transportation (auto insurance required).
• CPR certification required (Original Card).
• Level 2 criminal background check.

Aufgaben

• Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social factors.
• Develop and implement individualized care plans tailored to each patient’s needs and goals.
• Provide patient education and support to promote self-management and prevent complications.
• Coordinate care with physicians, therapists, and social workers to ensure continuity.
• Administer medications, treatments, and procedures as ordered by the physician.
• Document all patient care activities and communicate status to the team.
• Participate in quality improvement activities to improve outcomes and satisfaction.
• Maintain professional knowledge by continuing education and staying current in the field.
• Provide emotional support and compassionate care to patients and families.
• Adhere to all laws, regulations, and ethical standards of nursing practice.
