Job summary

Parx Home Health Care in Texas is seeking a Registered Nurse to join our home health team. The RN will deliver high-quality, coordinated care to patients in their homes and communities, following state and federal regulations and best practices.

The ideal candidate brings a Florida RN license, 1–3 years of home health or related experience, and a compassionate, patient-centered approach. Responsibilities include assessments, care planning, education, and collaboration with the healthcare team.

Qualifications

• Current and unrestricted RN license in Florida.
• 1–3 years of clinical experience in home health, hospice, or community health.
• Strong clinical skills and evidence-based nursing practices.
• Excellent communication and interpersonal skills with patients, families, and healthcare providers.
• Valid Florida driver’s license and reliable transportation (auto insurance required).
• CPR certification required (Original Card).
• Level 2 Criminal Background Check.

Responsibilities

• Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.
• Develop and implement individualized care plans tailored to each patient's needs and goals.
• Provide patient education and support to promote self-management and prevent complications.
• Coordinate care with physicians, therapists, and social workers to ensure continuity of care.
• Administer medications, treatments, and procedures as ordered by the physician or authorized provider.
• Document all patient care activities and communicate patient status and progress to the healthcare team.
• Participate in quality improvement activities to improve patient outcomes and satisfaction.
• Maintain professional knowledge and skills by attending continuing education and staying up-to-date with developments in the field.

Requirements

What we expect from you:

• Current and unrestricted RN license in the state of Florida
• Strong clinical skills and ability to perform evidenced based nursing practices.
• 1-3 years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health
• Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers
• Physical Exam and Two-Step TB Test or Chest X-Ray (no older than 6 months)
• Yearly Mandatory in-service
• Proficient in using electronic health records and other healthcare technologies
• Valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
• CPR certification required (Original Card)
• Level 2 Criminal Background Check
