Job summary

Baptist Home Health Care by BAYADA is seeking a compassionate Full Time Physical Therapist in Jacksonville, FL to start care at the patient’s home. The role requires home visits across the Jacksonville area and coordination with a multidisciplinary team to ensure safe, independent living for adult and geriatric clients.

The PT will perform assessments, develop treatment plans, and document progress using a tablet while maintaining BAYADA’s commitment to compassionate, reliable care.

Qualifications

• FL Physical Therapist license required.
• Minimum of 1 year recent PT experience.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.
• Florida Background Screening Clearinghouse is required.

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility, gait, balance, and strength.
• Implement wound prevention and healing interventions with evidence-based dressings.
• Develop and implement individualized PT care plans with activities.
• Continuously assess and revise the PT care plan with nursing input.
• Educate clients and families on rehabilitative care and safety for independent living.
• Accurately document observations and interventions using a tablet.
