Qualifications

• Proven experience as a Registered Nurse essential.
• Active RN license is required.

Responsibilities

• Record and assess a patient's medical history accurately.
• Monitor and record vital signs including blood pressure, temperature, and heart rate.
• Perform Start of Care (SOC) visits and ongoing patient evaluations.
• Develop and update patient care plans in coordination with physicians.
• Monitor patients' condition and identify progress or side effects of treatments.
• Administer medications, treatments, and IV therapy as prescribed.
• Educate patients and families and provide emotional support.
• Maintain accurate documentation and ensure compliance with home health standards.

Skills

Proven experience as a Registered Nurse

Ability to manage patient care independently and within a team

Familiarity with home health care standards and procedures

Strong understanding of confidentiality and nursing best practices

Excellent knowledge of medical terminology

Good knowledge of electronic charting systems

Strong clinical assessment and organizational skills

Excellent communication and interpersonal skills

Compassionate and able to handle stress

Active RN license
