Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health visits in Center Rutland, Vermont. You will join a multi-disciplinary team delivering skilled nursing and rehabilitative care to help clients remain at home with compassionate, reliable service.

You’ll perform head-to-toe assessments, manage acute and post-acute care needs, and support patients with medication reconciliation and wound care in diverse home environments.

Qualifications

• Current Vermont nursing license in good standing.
• Graduation from an accredited nursing program.
• At least one year of hands-on medical experience with adult/geriatric populations in acute care, home health, or similar settings.

Responsibilities

• Conduct head-to-toe assessments and identify changes in patient condition.
• Provide skilled nursing and rehabilitative care in home settings.
• Triage and independently manage urgent clinical situations in the home.
