Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in New Hampshire. You will provide skilled nursing and rehabilitative care to clients allowing them to remain at home, working with a multidisciplinary team dedicated to compassionate, reliable care.

You will perform head-to-toe assessments, manage acute and post-acute needs, and make real-time clinical decisions in patients' homes while documenting care using point-of-care technology.

Qualifications

• Current nursing license in good standing in New Hampshire.
• Graduation from an accredited nursing program.
• At least one year of hands-on medical experience with adult/geriatric populations in acute care, emergency, home health, hospice, ICU, med-surg, sub-acute or rehab.

Responsibilities

• Use head-to-toe assessment skills to identify changes in patient condition.
• Apply experience with acute and post-acute care.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct wound assessment, care, and documentation.
