Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in New Jersey. You will perform skilled nursing and rehabilitative care in the home, collaborating with a multidisciplinary team to help clients remain at home with compassionate, high-quality care.

You will assess patient conditions, manage urgent home visits, and conduct medication reconciliation and wound care documentation. BAYADA offers strong benefits and a supportive environment.

Qualifications

• A current nursing license in good standing in the state of New Jersey.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).

Responsibilities

• Conduct head-to-toe assessments and identify changes in patient condition.
• Triage and manage urgent home health situations independently.
• Perform medication reconciliation, teaching, and safety monitoring.
• Document comprehensive wound assessment, care, and documentation.
