Job summary

BayCare HomeCare in Largo, FL is seeking a compassionate Home Health Field RN to join our team and deliver top-quality care in patients' homes. You will work with a multidisciplinary team to coordinate and supervise care plans while promoting patient well-being.

The role is full-time (8:00 AM–5:00 PM) with occasional weekends and on-call duties; Florida RN license and BLS required, plus 1 year of RN experience. Bachelor’s degree preferred.

Qualifications

• BLS certification is a must.
• RN license valid in Florida required.
• Driver's license in Florida required within 30 days of hire.
• At least 1 year of RN experience required.
• Bachelor’s degree in Nursing preferred; 1 year Home Care preferred.

Responsibilities

• Oversee the activities of the multi-disciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.
