Job summary

Personal Touch Home Care is seeking a Registered Nurse for home health care in Jefferson County, IN, covering Madison and Hanover. Part-time or full-time positions are available with cases based in Jefferson County.

Qualifications

Graduate of an approved nursing program or Bachelor’s degree in Nursing.
Currently licensed to practice as a Registered Nurse.
Minimum 1 year of community health nursing or equivalent experience.
Knowledge of home care nursing practices and OASIS.
Complies with professional standards and practice.

Responsibilities

Assessment and Care Planning: conduct patient assessments and develop care plans.
Medication Administration: administer prescribed medications and monitor responses.
Wound Care: manage wound care including dressing changes and infection control.
Patient Education: educate patients and families on conditions, medications and self-care.
Monitoring and Evaluation: track progress and adjust care plans as needed.
Collaboration: coordinate with physicians, therapists, social workers, and aides.
Documentation: maintain medical records and nursing assessments.
Support and Advocacy: provide emotional support and uphold patient rights.
