Team Select Home Care is seeking a Registered Nurse (RN) to provide compassionate, one-on-one home health care for medically complex children and adults. You’ll work in patients’ homes, coordinating with physicians and therapists to deliver patient-centered care with a focus on safety and continuity.

We welcome passionate new grads and offer flexible scheduling, with a pay model that includes weekly pay and opportunities for ongoing education.

Competenze

• Active RN license in Florida.
• CPR certification required.
• Clinical experience preferred, but new grads are welcome.
• Excellent communication and critical thinking skills.
• Love for patient-centered home care.
• Calm, confident presence in routine and critical moments.

Mansioni

• Provide one-on-one home nursing care for medically complex patients, primarily at the patient’s home.
• Coordinate care plans with physicians, therapists, and other team members.
• Monitor patient progress and update care plans as needed.
• Educate and support families with in-home care techniques.
• Document visits and care notes in EMR.
• Collaborate with clinical supervisor and participate in case conferences.
• Be a trusted part of a long-term home health team focused on safety, comfort, and continuity.
