Qualifications

• BSN preferred or ADN.
• Must hold a valid and active RN license in Colorado.
• Prior home health, hospice, or community health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills with patients, families, and healthcare team.
• Ability to work independently and manage time effectively in home health.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide patient-centered nursing care in patients' homes and educate patients and families.
