The role involves direct patient care in home health and hospice, developing and evaluating care plans, and coordinating with a multidisciplinary team to deliver comprehensive services.

An active Virginia RN license is required, with BLS certification due within 90 days of hire; 1 year of nursing experience and home care/hospice background are preferred.
