Freedom At Home in Grand Rapids, MI seeks a dedicated Registered Nurse Case Manager to deliver high-quality home health care. You will provide professional nursing services to patients in their homes, collaborating with primary care physicians and other healthcare professionals to ensure exceptional patient outcomes.

The role requires an active Michigan RN license, CPR certification, and at least one year of nursing experience, preferably in home care.

Qualifikationen

• Minimum of one year of professional nursing experience, preferably in home care.
• Must have a criminal background check
• Must have current CPR certification.

Aufgaben

• Furnishes services requiring substantial nursing skill.
• Initiates preventative and rehab nursing procedures.
• Prepares and submits timely clinical and progress notes.
• Oversees coordination of patient services.
• Informs MD and staff of changes in patient condition or needs.
• Performs home safety checks and environmental assessments.
• Develops and submits Plan of Care (POC) for physician approval.
• Coordinates community services for safe home care.
