Job summary

Optimal Care in Grand Rapids, MI is seeking a Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes. You will travel daily to sites across Southern Grand Rapids and surrounding communities, starting care and guiding patients to discharge with individualized plans.

The role requires an active MI RN license, 1 year of RN experience, and strong clinical judgment. A $5,000 sign-on bonus is offered, with mileage reimbursement and comprehensive benefits.

Qualifications

ASN or BSN from an accredited nursing program.
Active RN license in Michigan.
Minimum one year of clinical RN experience.
Valid driver's license and reliable transportation.
Willingness to travel daily to service area.

Responsibilities
