Job summary

AccentCare Inc. seeks an RN Case Manager for Home Health in Marble Falls, TX. The role covers Marble Falls and surrounding areas with a standard Mon-Fri schedule, no on-call, and a per-point compensation approach.

The candidate will coordinate patient care, supervise aides, and ensure adherence to the Patient Care Plan. Qualified RNs must hold current Texas licensure, have at least one year of nursing experience, and CPR certification.

Qualifications

• Registered Nurse with current licensure to practice nursing in the state.
• Minimum one year of nursing experience.
• Maintains CPR certification.

Responsibilities

• Assess the nursing needs of assigned patients and implement the care plan.
• Evaluate and revise the care plan to ensure optimal patient outcomes.
• Provide direct care and supervise CHHAs and CNAs as needed.
• Document patient care and communicate changes to physicians.
