Job summary

Crossroads Hospice is seeking a Registered Nurse Case Manager to provide direct patient care in patients' homes or care facilities, building trusting relationships with patients and families, and serving as the primary communication link within our holistic care team.

The role emphasizes compassionate, individualized end-of-life care and collaboration with an interdisciplinary group to honor each patient’s wishes and dignity.

Qualifications

• 1 year of prior nursing experience.
• Active unencumbered state RN license.
• Current driver's license with auto liability coverage.
• Interest and ability to provide patient care in homes and facilities.

Responsibilities

• Provide direct patient care in patients' homes or care facilities.
• Act as the primary liaison between patients, families, and the interdisciplinary team.
• Promote compassionate end-of-life care and respect patient wishes.
• Communicate care plans and coordinate services with other providers.
