Job summary

Signature Healthcare at Home in Washington state is seeking a licensed Home Health Physical Therapist to deliver skilled PT services in patients’ homes within Pierce County (Tacoma/Lakewood area). This full-time, day-shift role offers a competitive hourly rate and mileage reimbursement, with Monday–Friday 8:00 AM–5:00 PM.

Qualifications

• WA State licensure as a Physical Therapist.
• CAPTE-accredited physical therapy degree.
• Two years of physical therapy experience (home health preferred).
• Strong verbal and written communication and organizational skills.

Responsibilities

• Provide rehabilitative physical therapy in patients' homes per physician plan of care.
• Assess, develop, and update the plan of care with patient and family involvement.
• Educate patients and families on exercises, precautions, and home program.
• Document clinical notes and progress; communicate with physicians and team members.
• Supervise Physical Therapy Assistants per policy and state regulations.
• Participate in discharge planning and program development.
