Job summary

The Christ Hospital Health Network’s Home Health Care is seeking a Physical Therapist to provide skilled services in patient homes and ensure quality documentation.

You will educate patients and families, perform OASIS assessments, supervises PTAs and aides, and coordinate with physicians and care teams. Valid Ohio PT license and driver’s license required; on-call and weekend shifts apply.

Qualifications

• Active PT license in the state of Ohio.
• Minimum one-year PT experience; home health experience preferred.
• Valid driver's license and auto insurance in your name.
• Ability to handle physical demands and communicate compassionately.

Responsibilities

• Provide skilled physical therapy as ordered by the attending physician.
• Observe and report patient reactions, progress, and changes to appropriate staff.
• Perform OASIS assessments to develop and adjust the care plan.
• Treat patients to relieve pain and restore function using therapeutic modalities.
• Educate patients and families on disease, injury, and therapy techniques.
• Supervise PTAs and home health aides as appropriate and document care.
• Complete clinical documentation within agency guidelines and timelines.
• Follow agency policies and communicate schedule changes to schedulers.
