Job summary

This is an in-person role requiring travel to patient residences within Thornton, Brighton, Aurora, Longmont, and Commerce City.

Qualifications

• Graduate from an approved physical therapy program.
• Licensed in the state(s) where practicing.
• Two years of experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Assist physician in evaluating function and plan of care.
• Prepare clinical and progress notes.
• Instruct patients and families in home programs and activities of daily living.
• Supervise PTA and HHA as assigned.
• Complete OASIS assessments, reassessments, and discharge planning.
