Human Touch Home Care is seeking a dedicated Physical Therapist Assistant to join our Colorado team. This role provides patient-centered home health therapy, under the supervision of a Physical Therapist, and involves collaboration with a supportive care team.

The ideal candidate will have completed a PTA program accredited by CAPTE and hold or be eligible for PTA licensure in Colorado, with at least 1 year of home health experience preferred.

Formación

• Must have completed a PTA program accredited by CAPTE or equivalent.
• Licensed in the state or eligible to take PTA exam.

Responsabilidades

• Performs services planned and supervised by the Physical Therapist.
• Assists in preparing clinical and progress notes; participates in patient/family education.
• Maintains documentation and adheres to agency policies.
• Coordinates therapy with PT and other health care personnel.
• Instructs patients in home programs and activities of daily living.
