Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist to provide home-based care, helping patients regain strength, mobility, and independence. You will work in patients' homes, plan and deliver prescribed therapy to restore function, and collaborate with a multidisciplinary team.

The role emphasizes compassionate care, documentation, and communication with physicians, managers, patients and families.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed PT treatment in patients' homes to restore function.
• Assess patients' physical strengths to guide goals.
• Develop rehab programs aligned with physician plans and patient goals.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient and family with required summaries.
